What happened

Slack couldn’t find, or couldn’t message, the channel or person in the action.

Wrong channel or user ID (use the ID, e.g. C0123ABCD or U0123ABCD, not the name).
The channel was deleted, or it’s private and the app hasn’t been added.
For direct messages: workspace settings don’t let the app DM that person.

Copy the ID from Slack (channel details → bottom of the “About” tab, or a person’s profile → “Copy member ID”) and update the action.
